Your First Visit to Dr. Rosenkilde
Please Bring With You the 5 sets of Forms that you printed from the section: "Patient Forms"
(please see bottom, left column.)
In addition, you will need to bring:
- Insurance card.
- Referral from primary physician, if indicated.
- Relevant previous medical records, particularly any neurological test results.
- Actual MRI or CT films, with reports, if the consultation concerns findings of such studies.
- Active case number for Worker's Compensation.
Please Notify When Scheduling Your Appointment:
- If your condition is related to work injury or motor vehicle accident.
- If you are seeking a second medical opinion from Dr. Rosenkilde.
- If you are seeking medical evaluation at the request of your attorney.
We Respectfully Request:
- Please arrive on time. You may be asked to fill out forms while in the waiting room, if you have not already done so.
- When visiting our office, please refrain from wearing perfume, lotion or aftershave as other patients may have severe reactions to these.
- During the consultation, one family member, companion, or friend may accompany the patient, as presence of multiple family members or friends has been shown to compromise, rather than facilitate the evaluation.
- Please turn off cell phones and beepers prior to your consultation.
