We apologize in advance for giving you homework before your first visit with us. If you would be so kind as to fill out the appropriate forms prior to your visit with Dr. Rosenkilde or Dr. Caporaso, your visit will be much easier.
In addition, it gives the doctors a chance to read your history before meeting with you.
Please, click on the underlined entries below to open up the necessary forms for your evaluation; there are currently FOUR sets underlined. You do not need to print the "Notice of Privacy Practices", provided that you read it and sign form that you have done so in our office. Your may need to install a Compatibility Pack to open the documents. Click on "Install" and follow the instruction on the screen. The installation process will last a few minutes.
You may fax the forms back to the office at 914-241-0413, or mail to us, if your appointment is in 2 or more weeks. The mailing address is:
