Instructions for Patients Before First Visit

We request that you bring:

Insurance information.

Referral from primary physician, if you belong to an HMO.

List of your current medications.

Relevant medical records, particularly any recent neurological    test results.

Actual MRI or CT films, with reports, if consultation concerns findings on such studies.

Active case number for W.C. conditions.

If your condition is related to work injury or motor vehicle accident, be kind to give notice with date of injury (you will need to fill out a special set of forms).

If you are seeking a second medical opinion from Dr. Rosenkilde, please tell us before the consultation.

If you are seeking medical evaluation at the request of your attorney, please let us know before the appointment is scheduled.

You will be asked to fill out several forms while in the Waiting Room.  Therefore, be sure to come on time.

While in our office, we kindly request that you avoid wearing perfumes, lotions or aftershaves as other patients may have severe reactions to these.  During the consultation, one family member, or friend, may accompany the patient.  Please turn off cellular phones and beepers while in the office.
